RENOWNED health reform advocate and prominent physician Dr. Tony Leachon has issued an urgent, high-stakes warning that the country’s Universal Health Care (UHC) system is on the verge of total collapse, facing a potential “flatline” by March 2027 if immediate executive and legislative interventions are not executed.
In a hard-hitting open letter addressed to President Ferdinand Marcos Jr., Dr. Leachon contrasted the administration’s current trajectory with the healthcare milestones of past presidencies—noting former President Benigno Aquino III’s passage of the 2012 Sin Tax Law and former President Rodrigo Duterte’s signing of the landmark UHC Law in 2019.
Dr. Leachon, a TOFIL Awardee in Medicine (2010), past president of the Philippine College of Physicians, and former special adviser to the National Task Force COVID-19, pointed to deep systemic roadblocks currently crippling the health sector.
Among the critical issues highlighted are:
- The PhilHealth Funding Crisis: A projected 2027 funding gap of ₱170 billion, alongside a proposed 2025 budget of only ₱74.45 billion against an estimated ₱244 billion requirement.
- Massive Deficits and Withheld Funds: An annual unfunded UHC gap scaling up to ₱541 billion, with ₱311.49 billion reportedly withheld from mandated health funds.
- Controversial Fund Transfers: The earlier transfer of ₱60 billion from PhilHealth, which was subsequently ruled unconstitutional by the Supreme Court.
- Heavy Out-of-Pocket Burdens: Out-of-pocket health spending reached 41.2% in 2025, keeping the Philippines among the highest in the ASEAN region for individual medical costs.
- Patronage Politics: Continued heavy funding for the Medical Assistance to Indigent Patients (MAIFIP) program over institutionalized, systemic health investments.
“Leadership is not measured by the races we attend abroad, but by the lives we save at home,” Dr. Leachon stated, criticizing executive priorities while urging structural reform.
He called on the administration to fully fund PhilHealth, integrate MAIFIP into a transparent system, reinforce public hospitals, empower frontline healthcare workers, and honor the promise of national healthcare equity.
